MG MGA - Coupe Door Pull

How is the coupe interior door pull fitted? I get that the 3 screws thru the leather attach to the inside metal of the door frame. But how is the handle accommodated past the door card and vinyl covering?

There is a recess in the top of the door card which allows the loop of vynide containing the handle to protrude to the inner face of the door card.

Thanks Dave. The reason for such a basic question...my car did not have one bit of original interior when I purchased it. There were MGB seats, a roadster dash (how does that happen), with the rest of the interior all just cobbled up. I have a lot of interior detail to sort out but I'm getting there!
