MG MGA - Lock and flat washers - placement order?

Hello all,
Is there a general rule about the order of placement of lock and flat washers on a bolt? Should the lock or flat washer be nearest the bolt head/nut?

The spring or lock washer goes next to bolt head or nut - the plain washer protects the component from being damaged.

The lock washer always goes on the nut side.

Unless you are screwing the bolt into a captive nut.

In which case as I said above, it goes directly under the head of the bolt, with the plain washer (if fitted) between the lock washer and the part being bolted down.

I don't think Richard would ask if you can put a lock washer or anything else under a captive nut since it's obviously impossible. I thought it was implied we were talking about a regular everyday bolt and nut situation.
